We’re glad to welcome you as a Be Well Clinic patient! Here’s what you need to know to help you best prepare for your first appointment. We’ve also included resources to help you understand our approach to health.
What to do before your first appointment:
Step 1: Complete Forms Ahead of Your Appointment
Once your appointment is scheduled, you will receive an email prompting you to fill out either 2 or 3 forms through Practice Better, our electronic medical record (EMR).

These forms must be filled out before your appointment or you will be asked to reschedule.
